CCQ announces an opportunity for health and/or social service providers to bid to operate the Integrated Team Care Program in Bundaberg.

The PHN invites all existing organisations who provide health and/or social services for Aboriginal and Torres Strait Islander people in the Bundaberg Local Government Area to participate in an open tender process.

What funding is available?

The PHN will be commissioning the Integrated Team Care (ITC) Program in the Bundaberg Local Government Area. The ITC Program is a Commonwealth funded program targeting chronic disease management care coordination and support to eligible Aboriginal and Torres Strait Islander clients. The Program was established to assist Aboriginal and Torres Strait Islander people with complex chronic diseases, who are unable to effectively manage their conditions, through access to one-on-one assistance provided by Care Coordinators.

The ITC Program is provided by a Care Coordinator and Aboriginal and Torres Strait Islander Outreach Worker who assist Aboriginal and Torres Strait Islander people to obtain primary health care as required, provide care coordination services to eligible Aboriginal and Torres Strait Islander people with chronic disease/s who require coordinated, multidisciplinary care, and improve access for Aboriginal and Torres Strait Islander people to culturally appropriate mainstream primary care.

The Care Coordinator has access to the Supplementary Services funding pool to expedite a patient’s access to an urgent and essential allied health or specialist service (including certain approved medical aids), where the services are not otherwise available in a clinically acceptable timeframe.

An Industry Briefing will be held on Thursday 1 November 2018 at 11.30am at the PHN office, Ground Floor, 205 Bourbong Street, Bundaberg.
