Heart Failure: an active role for GPs and patients

Early diagnosis of heart failure and effective management in primary care prolongs lives, keeps people out of hospital, and improves quality of life. The NPS MedicineWise program on heart failure has been developed in collaboration with the National Heart Foundation of Australia.

Managing heart failure is complex. High rates of mortality and hospital readmission make this a challenging condition for patients and their GPs. There are clear principals to support effective management in primary care. This education is part of a multifaceted program designed to:

• Support GPs in the early diagnosis and classification of heart failure.
• Provide clinical guidance on initiation and on-going management of medicines.
• Empower patients with effective self-management strategies to improve their quality of life.
