The Quality and Safety Standards (both NDIS and Aged Care) say if it isn’t documented it didn’t happen. Why is this so? Let’s put our investigative hats on and propose; if you have no evidence of something occurring it must be assessed as ‘didn’t happen’, ‘not in-place’ or ‘non conformance’. The practice standards require that providers keep records. You can’t measure what you can’t see. Therefore you don’t know if there are changes. Improvement, decline, change in need. It all has to be documented or noted. How would you feel if your GP, your Dentist or your Physio didn’t make notes about your care?
