CCQ covers a broad geographic area, with the vision to build healthy, resilient, connected communities.

The Primary Care Enhancement Program (PCEP) for People with Intellectual Disability.

This grant selected four lead PHNs with varied populations, geography and other demographics, to participate in the development of the PCEP. The objectives of the grant align with PHN aims to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care in the right place at the right time.


The Healthy Together Project has been developed to deliver better health outcomes and experiences for People with Intellectual Disability and their families.

The Healthy Together Project has been developed to support the PCEP program in delivering better health outcomes and experiences for people with intellectual disability and their families. Through improving the knowledge and skills of primary health care professionals in providing health care to people with intellectual disabilities, enhancing care pathways and increasing access to intellectual disability health resources for primary care professionals, people with intellectual disability, their families, carers and disability support providers.

Health professionals will soon be able to view intellectual disability specific resources to assist in better health care. These resources are suitable for allied and oral health and primary health care professionals.
