Advance Care Planning

As a part of the ‘Supporting Advance Care Planning in RACFs’ collaboration between Central Queensland, Wide Bay, Sunshine Coast PHN and the Brisbane South Palliative Care Collaborative (Queensland Health) and the, an ACP Train-the-Trainer Guide has been developed and is now available online

The Guide offers a package of evidence-based resources to support ACP Champions in RACFs to embed a seamless ACP program into routine clinical care. This supports high-quality, end-of-life care for residents in line with the Australian Aged Care Quality Standards.

An extensive process in networking and collaboration resulted in the development of 6 Roadmap posters and corresponding lanyard cards with vital contact details. Access the resources here.

Telehealth and Enhanced After Hours Support for Residential Aged Care Facilities project
Telehealth & Enhanced After Hours Support for aged care residents

As noted in the 2021 Federal Budget, PHNs have been funded to support RACFs to increase the availability and use of telehealth care for aged care residents. PHNs will help aged care facilities set up telehealth equipment so residents can consult health professionals virtually.
The Enhanced After Hours Support project will be focussed on implementing & improving after hours care planning, mapping of available after hours region-specific services, better collaboration between health professionals providing after hours care & RACF staff, and an education component to enhance best-practice clinical-decision making for RACF staff.
Residential aged care staff will have training in these services, so residents receive the best care possible with GPs, specialists and allied health professionals.
