The Yellow Envelope is a communication tool used in clinical handover when residents of aged care facilities are transferred to and from hospital. Its aim is to improve patient safety and the quality and continuity of care.
Yellow Envelope trial
The Yellow Envelope is being tried in the Sunshine Coast region from 18 October 2021 to 30 April 2022 across three residential aged care facilities. The facilities participating in the trial of the Yellow Envelope are Blue Care Caloundra, Blue Care Dicky Beach and Blue Care Warana. Throughout the trial, the Yellow Envelope tool will undergo a comprehensive audit and evaluation before it is rolled out across all facilities in Central Queensland, Wide Bay and Sunshine Coast.
